ACT Case Manager

The ACT Case Manager assists individuals with severe or chronic mental illness by using a team approach to provide treatment, rehabilitation and support services. This position provides, coordinates and delivers rehab services including medication training and psychosocial rehab. The ACT Case Manager assists individuals with obtaining transportation and / or translation for the purposes of accessing services. This position provides services to help individuals with mental illness maintain in the community, help prevent hospitalization, and help maintain mental health stability.
