A company is looking for a TOC Case Manager III. Key Responsibilities Provide member-centered, evidence-based care across multiple products Assess individual care goals and coordinate medical and supportive services Connect members with community resources to support their needs and objectives Required Qualifications Associate's or Bachelor's degree in Nursing 5+ years of clinical / acute care experience Experience with vulnerable and complex populations in clinical, home care, or telephonic environments Current, unrestricted RN license in the state of residence Certified Case Manager (CCM) preferred, or commitment to obtain within two years of hire
