A company is looking for a Health Navigator, Senior Care Outreach. Key Responsibilities Conduct educational calls to advise members on benefits and services, completing health needs assessments Encourage members to address gaps in care and assist with provider location and appointment scheduling Manage system work queues and facilitate research on inquiries related to member programs Required Qualifications Associates Degree in education, communication, or health-related field, or equivalent work experience 2 years of experience in a customer service support role Ability to work independently and in a team environment Knowledge of medical terminology Digital literacy assessment required
