POSITION DESCRIPTION :

Supports various healthcare programs and quality improvement initiatives in coordination with internal and external stakeholders. Analyzes and reports on data to improve patient care and quality outcomes, ensuring compliance with regulations and standards. Identify areas for improvement, develop and implement data-driven strategies, and collaborate with clinical staff and leadership to enhance performance. This role involves stakeholder engagement and coordination that supports the mission of SIHF Healthcare and facilitates the achievement of health center or department goals.

ESSENTIAL DUTIES AND RESPONSIBILITIES :

  • Implement assigned program activities, policies, and protocols, as directed by leadership .
  • Perform administrative support necessary to compile records for QA activities that may be confidential and sensitive in nature and help to ensure .adherence to regulatory standards, such as HRSA, HEDIS, Medicare, and Medicaid requirements, as well as HIPAA and organizational data privacy policies.
  • Collect, validate, analyze and report on data from various sources, including Electronic Health Records (EHRs), payor and claims data.
  • Develop and maintain close liaison with local agencies, businesses and civic organizations that are involved with the target population.
  • Work in collaboration with other medical / professional and ancillary staff as part of a patient-centered care team to develop, implement and support quality improvement plans and initiatives. .
  • Identify trends and patterns in data to pinpoint areas for improvement in patient care and clinical outcomes.
  • Assist with staff education on quality improvement principles and practices and assist in the development and review of quality assurance policies and procedures.
  • Attend meetings accordingly and provide regular feedback to leadership on daily activities, needs, successes, problems, etc. that pertain to the work area and cooperate on corrective action measures. Track the effectiveness of quality improvement efforts, monitoring progress towards goals and making adjustments as needed.
  • Prepare and present reports, dashboards, and visualizations to stakeholders, communicating findings and progress on quality initiatives.
  • Set a good example in the areas of professionalism, customer service, and adherence to SIHF Healthcare’s policies and procedures.

ADDITIONAL RESPONSIBILITIES :

  • Assist with the department’s community events and activities.
  • Interact and cooperate with other members of the management team to support the SIHF Healthcare mission.
  • Perform other duties as assigned by leadership.

  • Requires the ability to deal tactfully and courteously with staff, subordinates, patients and the general public.
  • Requires proficient personal computer skills (word processing, spreadsheets, graphics, routine, database, electronic mail) and analytical software and tools to analyze data.
  • Requires the ability to collect, organize, and manage data from various sources. Strong skills in creating reports, dashboards, and visualizations to communicate data findings.
  • Requires the ability to communicate effectively both orally and in writing and understand verbal and written instructions.
  • Requires the ability to lead by example.
  • Requires a neat and professional in appearance.
  • Requires the ability to maintain confidentiality.
  • Requires the ability to set boundaries as they relate to working with subordinates.
  • Requires the ability to think logically and to take the initiative to solve problems, develop solutions, and implement corrective actions.
  • EDUCATION AND EXPERIENCE :

    Bachelor's Degree and at least two (2) years experience in healthcare quality and / or data analysis . Master's degree is preferred. Candidate must have excellent writing skills and proficiency in computer systems such as Microsoft Excel, Word, PowerPoint in addition to familiarity with the health care needs of rural and urban communities.

    MEDICAL HOME STATEMENT :

    Be aware and understand expectations and responsibilities of the Patient Centered Medical Home. Understand role, complete relevant duties, and the requirement to maintain highest level of proficiency within scope, as part of this team-based approach in the patient care continuum.

    PHYSICAL DEMANDS :

    Light physical activity including being able to move and or lift up to 25 pounds; in addition to performing non-strenuous daily activities of an administrative nature. Local travel is required. A valid driver’s license and insured automobile are required.

    WORK SCHEDULE :

    Normal work hours for this position are Monday through Friday dayshift. Attendance at off site meetings will be required with no overnight stay. Occasional out-of-town travel may be required.
