We are seeking a full-time Care Navigator in Linn County to provide care coordination and supportive services to patients. The Care Navigator will utilize an intensive case management model to link patients to services such as childcare, transportation, housing, employment, and education in order to help patients to overcome barriers to their long term recovery.

Requirements :
Qualified candidates will have a bachelor’s degree in a human services field and one year of related work experience. This position requires a valid driver's license and the ability to pass an extensive background check.
Salary and Benefits
ASAC offers competitive compensation, pay starts at $22.71 with additional compensation for those with extensive experience.
