Registered Nurse - Case Manager
Salary Range : $53,000 annually
JOB SUMMARY : Provides health services to the youth of Hope. Collaborates with other youth service lines to ensure optimal care for all youth. Communicates health updates to the parents / guardians of the youth. Manages youth’s medications and treatments in the residential and school setting. Provides ongoing health services training to the residential and school staff.

WORKING CONDITIONS :
Work is performed primarily in an indoor office, school nurse office, or residential home, free from extreme weather conditions. Work will include walking, direct care of the youth, and the requirement to drive. Work may include a moderate amount of noise (business office with computers, phone and printers). Work will require working flexible hours and on-call hours.
KNOWLEDGE, SKILLS AND ABILITIES :
Ability to assess and refer youth for higher level of care when needed.
Ability to listen to and understand information and ideas presented through spoken words and sentences.
Ability to communicate and deliver staff education clearly and effectively, both orally and in writing;
Ability to navigate and utilize various software systems such as Microsoft Office and ERP systems.
Ability to plan and manage multiple tasks to meet specific deadlines.
Ability to handle confidential information with discretion.
Ability to analyze and prepare documents, reports and correspondence.
Ability to exercise sound judgement in making critical decisions.
Ability to handle difficult and stressful situations with professional composure.
Ability to establish, develop and maintain cooperative relationships with co-workers.
Skill in completing assignments accurately and with attention to detail.
