    This position encompasses managing a caseload of clients who are adults who experience Developmental Disabilities to live independently in their homes and to be integrated into their community by teaching, modeling, and encouraging skills and behaviors. The DSP helps each client in working towards their contract goals by providing a safe and supportive environment.

    Responsibilities

    • Manage a caseload of up to 12 clients
    • Problem-solving : coaching by providing constructive feedback in personalized ways, consistently training in appropriate behavior and living skills in a variety of ways for individual learners until competency is achieved.
    • Time management / Scheduling : planning to accommodate the needs and interests of multiple individuals with a variety of events five days a week, being aware of and adjusting the environment for sensory and mobility needs, scheduling for timely pickups and drop-offs of 1- 5 people and transporting them to events and areas of interest in the most efficient way possible considering different routes available.
    • Safety : Must be able to keep individuals safe at all times. This includes scanning areas for areas that might cause injury or behavior issues and problem-solving how to overcome the obstacles. Being trained in and monitoring health concerns such as diabetes, seizures, food allergies, and mental health.
    • Hosting events : Creativity with planning, preparing, and hosting events for large groups.
    • Additional Skills : Knowledge of different areas of the city, ability to search for and locate a variety of community activities, and create activities that meet the interests and abilities of participants.
    • Attend orientation training for new clients and those with renewing service agreements
    • Follow and uphold company policies & procedures.
    • Mandatory abuse reporter.
    • Transport clients safely, driving between 30 and 50 miles per day
    • Documentation

    • Record pick up and drop off time for each individual
    • Record daily activities on invoices.
    • Show disability and goal-related support on progress notes for all clients - four entries per outing
    • Complete Quarterly Progress Reporting for AC 1 : 1 Clients and addressing multiple Desired Outcomes with several Measurable Steps each month
    • Keep track of clients' hours and miles so as not to exceed them.
    • Complete the Expense Report and submit receipts by the 10th of each month.
    • Follow Trellis Policies and Procedures.
    • Use the Trellis employee website for documentation
    • Preferred Knowledge and Experience

    • Receives 24 hours of training annually on disability-related topics
    • OIS Training
    • Valid driver’s license and insurance.
    • 1 year of experience with people who experience a disability
    • Clean driving record.
    • CPR and First Aid Certificate
    • Basic computer skills
    • Job Type : Full-time

      Pay : From $21.00 per hour
