Summary :
The Case Manager oversees the provision of services and supports to individuals in accordance with each individual's identified outcomes as well as The Arc's Mission Statement and Guiding Values. The Case Manager is accountable to the person receiving support, their team, and the agency.
Essential Job Functions :
- Coordinates with agency licensing personnel for initial and annual licensing of Foster Care and Assisted Living Homes
- Assures placements are appropriate by agency standards and procedures, and licensing regulations
- Assures ongoing compliance with licensing regulations and HUD requirements
- Refers and assists individuals / families to appropriate community services / entitlements
- Develops and writes each person's Habilitation Plan
- Performs monthly checks of each residence and assigned vehicle to ensure they are in good condition and maintenance schedules as followed
- May provide direct care support, as directed by supervisor
- Makes reports in cases of known or suspected abuse and neglect under Alaska Statutes and The Arc of Anchorage Abuse / Neglect Policies
- Directs the activities of assigned staff or functions

High School Diploma or GED required. Associate degree in Human Services preferred.Two years previous social services work experience with case management responsibilities strongly preferredOne year in the field of developmental disabilities preferredClearances :
Background checks as defined by licensing agenciesTB Test, Valid driver's license, and proof of auto insurancePhysical Requirements :
