Case Manager – Contract – Oakland, CA – $23–$25 / hr

A community-focused organization in Oakland, CA is hiring a Case Manager for a contract role. This position supports individuals navigating healthcare and social services.

Key Responsibilities :

• Develop and implement individualized care plans
• Coordinate services and advocate for client needs
• Maintain accurate case documentation

Qualifications :

Pay Range : $23–$25 / hour

Contract Duration : 3–6 months

Pay Details : $23.00 to $25.00 per hour
