Job Summary : Job Summary :

The Community Health Worker / Community Justice Workers (CHW) plays a critical role in connecting individuals to healthcare, social services, and legal support through the Upstate Medical Legal Partnership (UMLP). This position works closely with healthcare providers, social workers, and legal professionals to address the social determinants of health, reduce health disparities, and improve overall community well-being. The CHW / CJW will engage directly with community members to provide health education, advocacy, and assistance in navigating healthcare, legal, and social service systems.

Key Responsibilities :

Community Outreach & Engagement :

  • Build trusting relationships with community members and patients / clients to assess their needs.
  • Conduct outreach activities to raise awareness about available healthcare, legal, and social services through the UMLP and its partners.
  • Participate in health fairs, community meetings, and educational workshops to increase awareness of UMLP services.
  • Health Education & Advocacy :

  • Identify barriers to care, including legal issues affecting health, and connect clients with appropriate resources.
  • Advocate for clients by helping them navigate healthcare systems and social services including assistance with applications to WIC, SNAP, Medicaid / Medicare, etc.
  • Provide referrals to UMLP partners for non HHLN SDOH needs such as housing insecurity and food insecurity,
  • Medical-Legal Partnership Support :

  • Work alongside legal professionals to identify health-related legal needs, such as housing instability, immigration challenges, access to benefits, and workplace rights.
  • Assist in screening patients for legal needs and making referrals to the UMLP legal team.
  • Educate clients on their rights and available legal protections that impact health outcomes.
  • Support healthcare providers in understanding legal barriers that affect patient care.
  • Case Management & Patient Support :

  • Assist patients with accessing healthcare services, benefits enrollment, and legal aid.
  • Provide follow-ups to ensure clients receive necessary support and services.
  • Help clients navigate medical-legal interventions, such as securing disability benefits or fighting housing evictions that impact their health.
  • Data Collection & Reporting :

  • Maintain accurate documentation of client interactions, services provided, and referrals made.
  • Assist in program evaluation, tracking outcomes, and identifying trends in social determinants of health.
  • Provide reports to supervisors and partners to improve service coordination.
  • Qualifications & Skills :

  • High school diploma or equivalent required; Associate’s or Bachelor’s degree in Public Health, Social Work, or a related field preferred.
  • Certification as a Community Health Worker preferred.
  • Experience in community outreach, case management, or social services preferred.
  • Knowledge of social determinants of health affecting vulnerable populations.
  • Strong interpersonal and communication skills, with the ability to work with diverse populations.
  • Ability to collaborate with healthcare providers, legal professionals, and social service agencies.
  • Bilingual skills are a plus.
  • Work Environment :

  • This position requires travel within the community for outreach, home visits, and collaboration with medical and legal teams.
  • Some evening and weekend work may be required.
