General Description :
The Behavioral Health Care Coordinator plays a vital role in the delivery of integrated behavioral health services by serving as a central point of coordination for patient care. This position facilitates seamless communication between patients, behavioral health providers, and medical teams, ensuring timely access to services and continuity of care. The coordinator supports patients in navigating the behavioral health system, reducing barriers to treatment, and promoting positive health outcomes through proactive care planning, follow-up, and resource connection.
The coordinator is responsible for managing and coordinating all behavioral health referrals by conducting brief needs assessments and scheduling patients with the most appropriate program, counselor, or psychiatric provider. This role includes providing timely and accurate community mental health referrals when needed and being prepared to respond to behavioral health crises in a calm and effective manner. Additional responsibilities include completing insurance verifications, and offering patients clear, upfront estimates of service costs prior to their initial appointments.
