THERAPEUTIC CASE MANAGER

Join our team and provide quality behavioral health care for children and families in Summit County!

There is a growing need for behavioral health services for our children and Child Guidance & Family Solutions is expanding to meet this need! We are looking for dedicated, compassionate people to provide mental and behavioral health support and wrap-around services to children, adolescents, and families through services delivered primarily in clients’ homes and in the community, including therapeutic behavioral services, community psychiatric supportive treatment, and therapeutic case management. The Therapeutic Case Manager helps to link families to additional community services, helps clients reinforce skills recommended by their therapist, and coordinates closely with other care team members, school personnel, court, and other external entities.

Responsibilities :

• Provides TBS and CPST services in clients’ homes and in the community.
• Identifies client needs and carries out referrals to both internal and external resources.
• Provides education to parents / guardians and school personnel to effectively manage mental and behavioral concerns for client; assists parents / guardians with advocacy for client’s needs in school settings.
• Coordinates and collaborates with primary therapist to modify treatment plans as needed to ensure clients are meeting individualized goals.
• Maintains regular ongoing contact with parents / guardians; collaborates with client collaterals (teachers, probation officers, protective service workers, etc.) via telephone or community meetings when necessary.
• Runs skills-based groups covering a variety of mental health related topics.
• Provides transportation for clients when appropriate.
