The Community Health Worker (CHW) is a frontline public health professional who supports member benefits, care coordination, chronic condition management, patient outreach, and quality improvement. CHWs use evidence-based navigation services to improve patient outcomes, especially for those facing psychosocial barriers. They build trusting relationships by leveraging their understanding of the community's language, culture, and socioeconomic background. CHWs connect members to resources, ensure access to care, and help them make informed decisions. They also contribute to program development and expand member access across PacificSource regions.

SUCCESS PROFILE
Work Experience : A minimum of three (3) years of experience in community-based healthcare, public health, or social services. Experience in health insurance, familiarity with chronic disease management, health plan benefits, and STARS / QIM measures preferred.
Education, Certificates, Licenses : High school diploma or equivalent required. Certification as a Community Health Worker (CHW) strongly desired at time of hire. CHW certification required within one year of hire. Additional training in social work, public health, or a related field is preferred. Valid driver's license and automobile that is insured.
Knowledge : Medical terminology. Proficient in Microsoft Office, including Word, Excel, PowerPoint, Medical management software (Excellent verbal and written communication skills and ability to work independently as well as to work effectively on a team. Excellent working knowledge of how to access community resources and healthcare system. Bilingual preferred.

Environment : Work inside in a general office and clinical settings with ergonomically configured equipment. Travel for community outreach or home visits. Travel is required approximately 50% of the time. May be required to use personal vehicle for work-related purposes and to meet with members in the community setting. May need to work outside normal work hours.
