The Community Health Worker (CHW) is a frontline public health professional who supports member benefits, care coordination, chronic condition management, patient outreach, and quality improvement. CHWs use evidence-based navigation services to improve patient outcomes, especially for those facing psychosocial barriers. They build trusting relationships by leveraging their understanding of the community's language, culture, and socioeconomic background. CHWs connect members to resources, ensure access to care, and help them make informed decisions. They also contribute to program development and expand member access across PacificSource regions.

Qualified candidates must be acceptable to the following available work schedule :

  • Monday - Friday 8 : 00am to 5 : 00pm

Essential Responsibilities :

  • In coordination with the member's case manager, identify and screen members for coordination, care management services, and social determinants of health resource needs.
  • Assist members in navigating their health plan benefits, including coverage details and financial assistance programs (e.g., transportation, medication support), and educate them about treatment options.
  • Assist members in addressing and overcoming barriers related to housing, food insecurity, language needs, health literacy, financial constraints.
  • Collaborate with the care team (e.g., providers, case managers) to identify high-need or at-risk members.
  • Facilitate referrals to internal and community resources, ensuring timely follow-up.
  • In coordination with the member's case manager, develop and implement goals and / or plans created by providers, tracking member progress and coordinating necessary adjustments.
  • Work with clinical and quality teams to address care gaps, including preventive screenings, annual wellness visits, and chronic disease monitoring.
  • Document improvements in STARS / QIM metrics, providing feedback to optimize ongoing initiatives.
  • Serve as liaison between members, providers / agencies, and other community partners focused on addressing social determinants of health (SDOH).
  • Share insights on community-specific challenges or cultural nuances with the multidisciplinary team to enhance service delivery.
  • Participate in team meetings, offering recommendations based on firsthand experiences with members and community partners.
  • Maintain accurate records of member encounters, interventions, and outcomes in relevant systems (e.g., EMR, care management software).
  • Create reports on member progress, outreach successes, and challenges, informing leadership and refining strategies across PacificSource regions.
  • Engage members using motivational interviewing and patient-engagement techniques to support them in achieving optimal health outcomes by effectively utilizing their benefits and services.
  • Participate in evaluating CHW services and programs, including individual and community assessments.

    • Supporting Responsibilities :

  • Assist internal departments to meet quality measures as outlined by government regulations.
  • Assist in developing and monitoring departmental performance metrics, procedures, reports and projects.
  • Participate in team, department, company, and community-related projects / committees as requested.
  • Meets department and company performance and attendance expectations.
  • Follow the PacificSource privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
  • Perform other duties as assigned.

    • SUCCESS PROFILE

    Work Experience : A minimum of three (3) years of experience in community-based healthcare, public health, or social services. Experience in health insurance, familiarity with chronic disease management, health plan benefits, and STARS / QIM measures preferred.

    Education, Certificates, Licenses : High school diploma or equivalent required. Certification as a Community Health Worker (CHW) strongly desired at time of hire. CHW certification required within one year of hire. Additional training in social work, public health, or a related field is preferred. Valid driver's license and automobile that is insured.

    Knowledge : Medical terminology. Proficient in Microsoft Office, including Word, Excel, PowerPoint, Medical management software (Excellent verbal and written communication skills and ability to work independently as well as to work effectively on a team. Excellent working knowledge of how to access community resources and healthcare system. Bilingual preferred.

    Environment : Work inside in a general office and clinical settings with ergonomically configured equipment. Travel for community outreach or home visits. Travel is required approximately 50% of the time. May be required to use personal vehicle for work-related purposes and to meet with members in the community setting. May need to work outside normal work hours.

    • Physical Requirements : Stoop and bend. Sit and / or stand for extended periods of time while performing core job functions. Repetitive motions to include typing, sorting and filing. Light lifting and carrying of files and business materials. Ability to read and comprehend both written and spoken English. Communicate clearly and effectively.
