Position : Full-time (37.5hrs / week)
Schedule : Mon-Fri, 8 : 30AM-4 : 30PM, Weekend On-Call Rotation
Compensation : $19.00

Position Summary :
The Care Coordinator works closely with clients, direct care staff and Registered Nurse (RN) team to create, monitor and adjust care schedules that ensure needs are met as described in Community Living Alliance (CLA) client care plan.

Creates schedules for client’s care needs based on plan of care, client preference, and authorized hours of service.Uses Community Living Alliance (CLA) scheduling software to match available caregivers to client’s schedules, document communication, work schedules and respond to time of requests.Communicates with caregivers, RNs, and clients including verbally, in writing, and with use of scheduling software, Electronic Visit Verification (EVV) system, or other electronic methods to make appropriate scheduling matches.Monitor and respond as necessary to CLA call-in system during normal business hours.Support, participate, and ensure care coverage during non-business hours related to on call rotation determined by team.Support onboarding, training, and ongoing mentorship to CLA direct care staff (i.e. Home Care Specialist, Personal Care Workers)Support, participate, and ensure care coverage during non-business hours related to on call rotation determined by team supervisor. On Call Hours Rotation may include :Mon-Fri 6 : 00am-8 : 30am 4 : 30pm-9 : 00pm
Weekends and Holidays 7 : 00am-9 : 00pm
Education / Experience and Requirements :
Required
18+ years of ageTwo (2) years of experience in providing direct personal cares, required.Also considered, clinically based training programs in lieu of experience.
High School Diploma, required.Valid driver's license, reliable transportation, and current auto insurance to serve clients throughout Dane CountyWork around environmental hazards such as odors, second-hand smoke, animals, cleaning supplies, adverse living conditions, etc.Satisfactory Results of a Background Check.Available to work on a weekend rotation.Authorized to work in the United States.Capable of lifting / moving / pushing / pulling 35 poundsPreferred
