Northbridge is now hiring a full-time Case Manager to serve individuals with intellectual and developmental disabilities.
The ideal candidate will be 21 years of age or older, have a vehicle with valid registration and insurance. Hold a valid Alaska State Driver's License and have at least two years with a good driving record. Care Coordinator training and / or experience. A minimum of two years in the Human Services field. Knowledge of Medicaid waivers. The ability to pass a State and Federal Background Check.
Job Duties :
Review case notes for quality and timeliness
Communicating effectively with individuals’ teams
Participating in individual Support Plan meetings and ensuring quality documentation is provided to pertinent team members
Monitoring and updating Behavior Support Plans, Assisted Living Plans, Plan of Care, etc. as needed or required
Interviewing applicants
Ensuring compliance with state and federal regulations regarding documentation, individual files, trainings, etc.
Maintain an organized work area
Conducting home visits with individuals
Understanding of state regulations.
