Job Summary :

We are actively seeking a compassionate and dedicated Hospice RN Case Manager to join our team in Pahrump, NV. The ideal candidate will provide high-quality nursing care to patients in a hospice setting in group homes, assisted living facilities or private residences, ensuring comfort and support for patients and their families during end-of-life care. The Hospice RN Case Manager will be responsible for coordinating patient care, developing care plans, and collaborating with interdisciplinary teams to deliver comprehensive services.

Key Responsibilities :

• Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
• Provide direct nursing care to patients, including medication administration, symptom management, and emotional support.
• Educate patients and families about the hospice process, available services, and end-of-life care options.
• Coordinate with physicians, social workers, and other healthcare professionals to ensure holistic care for patients.
• Monitor patient progress and adjust care plans as necessary to meet changing needs.
• Maintain accurate and timely documentation of patient care and progress in compliance with regulatory standards.
• Participate in interdisciplinary team meetings to discuss patient care and improve service delivery.
• Advocate for patients and families, ensuring their preferences and values are respected in care decisions.
• Two weeks of On-Call required as well.
