Job Summary :
We are actively seeking a compassionate and dedicated Hospice RN Case Manager to join our team in Pahrump, NV. The ideal candidate will provide high-quality nursing care to patients in a hospice setting in group homes, assisted living facilities or private residences, ensuring comfort and support for patients and their families during end-of-life care. The Hospice RN Case Manager will be responsible for coordinating patient care, developing care plans, and collaborating with interdisciplinary teams to deliver comprehensive services.
Key Responsibilities :
- Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
- Provide direct nursing care to patients, including medication administration, symptom management, and emotional support.
- Educate patients and families about the hospice process, available services, and end-of-life care options.
- Coordinate with physicians, social workers, and other healthcare professionals to ensure holistic care for patients.
- Monitor patient progress and adjust care plans as necessary to meet changing needs.
- Maintain accurate and timely documentation of patient care and progress in compliance with regulatory standards.
- Participate in interdisciplinary team meetings to discuss patient care and improve service delivery.
- Advocate for patients and families, ensuring their preferences and values are respected in care decisions.
- Two weeks of On-Call required as well.

Requirements : Qualifications :
Current Registered Nurse (RN) license in the state of practice.Bachelor’s degree in Nursing (BSN) preferred.Previous experience in home health, hospice, palliative care, or a related field is highly desirable.Strong clinical assessment and critical thinking skills.Excellent communication and interpersonal skills to effectively interact with patients, families, and team members.Ability to work independently and manage time effectively in a fast-paced environment.Compassionate and empathetic approach to patient care.Must have reliable transportationSkills :
