About the Role :

The Home Health Registered Nurse Supervisor / Case Manager plays a critical role in ensuring the delivery of high-quality healthcare services to patients in their homes. This position involves overseeing nursing staff, coordinating patient care plans, and ensuring compliance with healthcare regulations and standards. The supervisor will work closely with patients, families, and interdisciplinary teams to assess needs and implement effective care strategies. By fostering a supportive environment, the supervisor will enhance patient outcomes and satisfaction. Ultimately, this role is pivotal in bridging the gap between hospital care and home health services, ensuring continuity of care for patients.

Minimum Qualifications :

• Current and valid Registered Nurse (RN) license in the state of practice.
• Bachelor's degree in Nursing (BSN) or higher.
• A minimum of 3 years of nursing experience, with at least 1 year in a supervisory or management role.
• Proven experience in nursing administration, preferably in home care, nursing home, and hospice setting.
• Strong knowledge of Medicare regulations and compliance standards.
• Excellent leadership skills with the ability to manage diverse teams effectively.
• Proficient in developing systems that improve patient care delivery and outcomes.
• Exceptional communication skills, both verbal and written.
• Ability to analyze data and make informed decisions based on patient needs.
