Description
Performs basic screening procedures for primary care clinic as assigned. The Patient Care Associate also performs other tasks as assigned to assist in the well functioning of on and off site clinics.
The Patient Care Associate consistently demonstrates competency in up to 50% (in any combination) of the following Advanced-Level Skills as assessed at time of hire and annually thereafter : All essential PCA functions in Pediatrics, all essential PCA functions in OB / GYN; all essential functions in Family Practice; all essential PCA functions in Podiatry; EKG operational assist; Diabetes Total Care program assist; Certified Diabetic Educator assist; simple procedure such as toe nail removal, skin tag and wart removal assist; advanced procedures such as Colposcopy, LEEP and endometrial biopsy assist; dressing changes assist, family planning assist; mobile van assist; all essential functions in registration; travels to other sites.
Primary Functions : 1.Prepares physical set-up of clinics; cleans up after clinic, and restocks / straightens medical supplies as indicated.
- Completes, records, and graphs client's length / height, weight, head circumference, and vital signs per applicable procedures and protocols.
- Completes and records Waived Testing as applicable for IDPH and VNA Health Center procedures.
- Completes quality control, checks on all CLIA waived equipment and other QC checks as determined by the Associate Director of Health Center or Health Center Coordinator.
- Completes sterilization of instruments, autoclave maintenance and documents the sterilization process per applicable procedures and protocols.
- Completes applicable laboratory requisitions and logs for tests completed (e.g. lead, pap, GC).
- Prepares laboratory specimens for pick-up or mailing per applicable IDPH and VNA procedures.
- Directs client or parent / guardian in completion of clinic forms (e.g. health history, developmental, dietary...) as applicable.
- Notifies staff nurse of problems or alterations in client status.
- Assists staff nurse, clinic physician or Nurse Practitioner as requested.
- Provides translation services and support for the health center patients, clients and staff.
- Specific to Translation : Translates messages simultaneously or consecutively, maintaining message content, context, and style as much as possible. Strives to make the communication as clear as possible, and is able to adapt translation to client’s cognitive and grade level. Relays information, does not initiate or change it, and does not counsel, advise or interject personal opinions. Identifies and resolves conflicts related to the meanings of words, concepts, practices, or behaviors, and is able to translate jargon and acronyms effectively. Remains with the client and Provider during the appointment until the Provider has completed the visit when translation services are required.
- Understands and practices Exposure Control Procedures.
- Maintains current CPR certification.
- Respects client's privacy and rights and maintains client confidentiality.
- Indicates interest in personal growth and VNA mission / goals and participates in staff meetings / inservices.
- Ability to work all clinical hours as assigned and travel to assigned clinics as needed
- Able to work efficiently and effectively in the Electronic Medical Records System.
- Familiarizes and complies with all VNA policies and procedures.
- Meets standards of behavior expectations.
- Follows established guidelines for use and / or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer immediately. Failure to comply with HIPAA policies and procedures will result in disciplinary action, up to and including termination of employment.

Compensation : Hourly Range : $20.00 - $21.00 Depending on education.
Shift Differential : $3.00 for hours worked after 5 : 00 pm and any hours on a Saturday or Sunday.
Incentives : Earn up to $500.00 quarterly based upon meeting productivity goals.

Qualifications
Requires high school diploma or G.E.D.Provides proof of program completion from an approved Certified Nurse Assistant (HCWR Competency)Maintains CPR card.Ability to type and basic computer skills a minimum.Effective communication skills, including neat and legible hand writing / printing.Ability to translate medical information effectively.Bilingual in Spanish / English as follows : Reading - Preferred. Written – Preferred. Speaking – Required.Health policies and requirements met (See Personnel Policies).Essential Functions / Physical Requirements : 1. Ability to communicate effectively verbally and in writing.
2. Tactile, visual, auditory and olfactory senses intact, including the ability to distinguish colors.
3. Manual dexterity sufficient to perform primary functions of CNA and operate a computer.
4. Ability to deal with high levels of stress.
5. Ability to bend frequently and lift up to 50 pounds, 25 pounds on a frequent basis.
6. Ability to sit or stand for prolonged periods.
