A company is looking for a Care Management Support Coordinator II. Key Responsibilities Perform outreach to members via phone to assist with care plan next steps and resources Connect members to health plan and community resources for quality customer care Document and maintain non-clinical member records in compliance with regulations Required Qualifications High School diploma or GED required 1 - 2 years of related experience preferred Knowledge of health plan activities and resources Understanding of Social Determinants of Health (SDOH) Ability to comply with policies and standards
