Grandma's Place is committed to supporting individuals with chronic persistent mental illness, who are high functioning with intellectual disabilities, have a brain injury, and who may or may not have substance abuse / misuse barrier. To assist our clients with developing independent living skills in their own apartment and to help them lead a more self-directed life.
Position Description
The Designated Coordinator plays a crucial role in overseeing an Integrated Community Support (ICS) setting. Grandmas Place ICS settings are non 24 hour settings and allow individuals to receive 1 : 1 services and activities while living in their own apartment building, allowing for greater independence and growth. Each setting provides services to 10 to 15 clients and services are provided by 8-10 staff members. Designated Coordinators meet with clients and their supportive teams to develop and revise plans of care to meet client needs. They get to develop and train employees and build strong relationships with clients, while providing direct support services to help them lead more independent lives.
Position Responsibilities and Duties
Participate as a part of the Service Management team to support the Grandmas Place in achieving its vision and goals.
Living Grandma's Place core values, following policies and procedures, and abiding by federal, state, and local regulations.
Provide leadership, management, and accountability for an integrated community support setting
Oversee the delivery and evaluation of services for 10 -15 clients
Provide oversight of the license holder's responsibilities assigned in the clients coordinated service and support plan
Take the action necessary to facilitate the accomplishment of the outcomes according to the client support plan
Evaluate the effectiveness of service delivery, methodologies, and progress on the person's outcomes based on the measurable and observable criteria
Communicating and meeting with clients and their support team members
Oversee the hiring, training, supervision, support, and evaluation of 7 to 10 employees
Provide instruction and assistance to direct support staff implementing the coordinated service and support plan and the service outcomes, including direct observation of service delivery sufficient to assess staff competency
Other duties as assigned
Minimum Qualifications

Be 18 years of age or older
Completed and acceptable background study
Completed and acceptable TB screening
Ability to effectively communicate using English in both verbal and written communications.
Proven success making sound independent decisions.
Must be able to effectively navigate the internet, Office, and use email
Must possess exceptional organizational skills, attention to detail and able to prioritize
Meet the educational qualifications of a designated coordinator as identified my MN Statutes 245D.081
Preferred Qualifications
Previous supervisory experience in a licensed 245D setting overseeing services to individuals with mental illness, brain injury and intellectual disability diagnosis
Experience supervising staff and leading effective teams
Experience developing, monitoring and implement CSSP-As
Previous experience administering medication in a health care or human services setting
Experience overseeing budgets
Positions Supervised by : Director of Operations
Position Supervises : Direct Support Professionals (8-10)
Hours Per Week : 35-45 hrs. per + on call as needed
Status : Exempt
Wage : $45,700 to $60,000 annual salary
