Transitions Of Care Pharmacy Coordinator
Works under the supervision of the Director of Pharmacy and Assistant Director of Operations to develop and manage transitions of care coordination at SVH. The Transitions of Care Pharmacy Coordinator works in collaboration and continuous partnership with chronically ill or "high-risk" patients identified by the organization. The coordinator will work with family / caregiver(s), clinic / hospital / specialty providers, staff, and community resources in a team approach to promote effective patient transitions of care. This pharmacist will also direct and evaluate the Clinical Pharmacy Staff in transitions of care. Responsible for optimization and utilization of cost effective drug therapy in the hospital.
Responsibilities include :
- Develops, implements, and manages a pharmacy based medication reconciliation program.
- Assists the departmental staff pharmacist on patient medication transitions of care.
- Imparts therapeutic and drug information to patients and health care professionals.
- Maintains patient records to monitor drug therapy for appropriateness and safety.
- Participates in clinical patient rounds.
- Develops, plans, implements, and maintains clinical programs and projects promoting patient care.
- Serves on the hospital's Pharmacy and Therapeutics Committee and other clinical and performance improvement committees.
- Develops procedures and oversees the process of monitoring adherence to medication care plans.
- Facilitates patient access to appropriate medical and specialty providers.
- Educates patients and family / caregiver(s) about relevant community resources.
- Facilitates and attends meetings between patient, family / caregiver(s), care team, payers, and community resources.
- Cultivates and supports primary care and specialty provider co-management.
- Assists with the identification of "high-risk" patients.
- Attends all Care Coordinator training courses / webinars and meetings.
- Provides feedback for the improvement of the Care Coordination Program.
- Develops and manages pharmacy based medication reconciliation program. Reduces patient emergency room utilization and hospital readmissions. Increases continuity of care by managing relationships with tertiary care providers, transitions-in-care, and referrals. Increases patients' ability for self-management and shared decision-making. Connects patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs.
- Performs other duties as assigned.
Education : Postgraduate degree in Pharmacy RPh or PharmD required.
Licensure : Current California Pharmacy License required. Current BLS / Healthcare Provider status as per American Heart Association standards required.
Experience : Three (3) years' experience in a clinical or community resource setting required. Care coordination experience is desirable. Transitions of care residency training preferred.
The hourly rate for this position is $76.24 - $95.30. The range displayed on this job posting reflects the target for new hire salaries for this position.
