Key Responsibilities Assess, plan, implement, and coordinate case management activities for members to evaluate their medical needs Utilize clinical tools to conduct evaluations and apply clinical judgment to address health and social indicators impacting care planning Collaborate with stakeholders to overcome barriers and enhance member engagement through motivational interviewing Required Qualifications Registered Nurse with an unrestricted nursing license and multi-state / compact privileges 3+ years of clinical / nursing practice experience 1+ year of medical case management, care coordination, or discharge planning experience 3+ years of experience with personal computers and MS Office Suite applications High-speed internet access for telework
