Washington RN Case Manager

A company is looking for an RN Case Manager to coordinate patient care and advocate for patients across various healthcare settings. Key Responsibilities Conduct intake calls and ongoing patient outreach to monitor care plans and address new concerns Provide care coordination for complex patients and develop plans of care with the care team Support patients during care transitions and collaborate with multidisciplinary teams to ensure comprehensive care Required Qualifications Current RN License in good standing in the state of Washington Minimum associate degree in nursing required, bachelor's degree preferred Minimum of 3 years' experience in a clinical setting, with at least one year in case management Certified Case Manager (CCM) certification or commitment to complete when eligible Geriatric care experience highly desired
