Key Responsibilities :
- Conduct comprehensive physical and psychosocial assessments of patients.
- Develop and implement care plans with therapeutic goals in partnership with patients and their families.
- Monitor patient safety continuously and react appropriately to changes in patient condition.
- Ensure accurate and timely documentation of all services provided in compliance with regulations.
- Provide orientation and supervision to Home Care Aides in accordance with agency policies.
- Maintain high standards of patient satisfaction and quality care while managing assignments efficiently.
- Engage in ongoing professional development through training and education.
