Location : Visits across Orange County, Los Angeles and Inland Empire

Schedule : Full-Time

Compensation : $95,000- $115,000 annually, commensurate with experience

Position Summary : We are seeking a compassionate and skilled Registered Nurse to join our Palliative Care Program. This role focuses on providing holistic, patient-centered care in various settings, including patients' homes, assisted living facilities, and skilled nursing facilities. You will play a vital role in delivering high-quality palliative care, supporting patients and their families during challenging times.

Key Responsibilities :

• Creatively manage resources and workflows to triage patient needs and address clinical demands.
• Coordinate with the interdisciplinary palliative care team and referring providers to implement effective care plans.
• Facilitate connections with complementary therapies (e.g., PT / OT, massage, music therapy).
• Assist with non-pharmacological interventions to alleviate symptoms and enhance psychosocial coping (e.g., meditation, Reiki, journaling).
• Support patients and families in legacy work to aid end-of-life coping.
• Ensure continuity of care and consistent team presence across rotations.
• Engage in advanced care planning discussions and assist with completing advance directives.
• Deliver nursing care in patient residences and document assessments within the RN scope of practice.
• Act as a resource for patient care units, outpatient clinics, and staff regarding palliative care.
• Provide education to staff, patients, and families on symptom management, coping strategies, and end-of-life care.
