RN Hospice Admissions - Weekends - Broward County

Overview

Pay range :   $33 / hr. - $43 / hr. based on experience

Hospice nurses provide end of life care to help ensure the comfort and quality of life for their patients.  They primarily work in collaborative, team-driven environments that require frequent communication with their patients, families and other caregivers.  The ideal candidate should be detail-oriented, patient and calm.  Hospice nurses are caring and kind individuals who possess a special hospice heart who can help our patients celebrate the rest of their life’s journey.

Responsibilities

Friday / Saturday / Sunday 9am - 9 : 30pm

Duties and Responsibilities

• Carries primary responsibility for evaluation of all patients referred for hospice services.
• Is able to articulate all components of the organization's continuum of care and refer patients and families to those as necessary.
• Assists referring physicians and / or his / her designated liaison with coordinating a plan of care / treatment for hospice appropriate patients and their families.
• Serves as the liaison between our company and physicians, hospitals, long-term care facilities, and other health care professionals.
• Actively markets, through education, the hospice concept to all health care professionals and providers, as well as the community at large.

Qualifications

RN degree from a college or university, accredited by the Accreditation Commission for Education in Nursing and / or the Commission on Collegiate Nursing Education.

Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.

Able to communicate effectively with other Hospice care personnel.

Bilingual (English / Spanish) preferred.
