Responsibilities for Case Manager :

Experience as a Social Worker, Registered Nurse or Licensed Practical Nurse.

Experience as a Case Manager or Reimbursement / Utilization Coordinator in a SNF setting required.

Discharge planning experience is required.

Ability to work collaboratively and effectively with the interdisciplinary team members, residents / patients, families, and external care team members.

Skilled in assessment, planning, and managing residents utilization management of care

Ability to organize and prioritize work and to support the resident / family / caregiver needs

Excellent verbal and written communication skills, expressing self in a clear, concise, and professional manner

Qualifications for Case Manager :

Must have a valid driver’s license since traveling is a necessity

RN / LPN / LSW preferred

Associate's degree in an appropriate field. Bachelor’s degree preferred.

Must be willing to work with multiple residents at once
