The Hospice RN Case Manager is responsible for coordinating and managing the care of patients in a hospice setting, providing skilled nursing services and overseeing the implementation of care plans. This role requires strong clinical knowledge, excellent communication skills, and the ability to manage patient care autonomously while collaborating with an interdisciplinary healthcare team.

This is a field-based position ideal for a nurse who thrives on autonomy, flexibility, and meaningful patient relationships.

Primary Responsibilities and Duties :
- Conduct initial and ongoing assessments, including OASIS documentation
- Create and manage individualized care plans in collaboration with an interdisciplinary team
- Coordinate with physicians, therapists, aides, and families to ensure cohesive care
- Supervise and support field staff (RNs, LPNs, Therapists, HHAs)
- Maintain timely, accurate documentation and communicate effectively with care teams
- Conduct comprehensive patient assessments to identify healthcare needs, develop individualized care plans, and update plans as needed.
- Provide skilled nursing care, including pain and symptom management, medication administration, and patient education, in accordance with the care plan and physician orders.
- Communicate effectively with patients, families, physicians, and other members of the healthcare team to update them on patient progress, discuss treatment plans, and address any concerns.
- Monitor patients' physical, emotional, and spiritual well-being, ensuring their comfort and dignity during end-of-life care.
- Ensure compliance with regulatory guidelines, documentation requirements, and facility policies and procedures related to hospice care.
- Provide bereavement support and resources to patients' families and caregivers as needed.
- Participate in team meetings to discuss patient care plans, progress, and any barriers to achieving patient goals.
- Perform all other duties assigned by the Hospice Director or designated supervisor.
