Definition
Definition : Under general supervision from the Community Health Services Manager in the Health Services Division of the Health & Human Services (HHS) Department, provides community health nursing services to SRPMIC Community Members with the goals of promoting and restoring health to the Salt River Pima-Maricopa Indian Community, fostering wellness and healthy living and addressing health disparities through community-driven initiatives at River People Health Center. This job class is treated as FLSA Exempt.

Examples of Tasks
Assessment
- Monitor and promote health outcomes for referred and enrolled clients in the community health program
- Identify community health problems through comprehensive in-home assessments from a biopsychosocial perspective to support chronic disease management, decrease unnecessary emergency room utilization, and support patient adherence towards self-management.
- Create in collaboration with the client and intervention plan to address individual, health, safety and quality needs towards health and disease management.
Direct Care
Medication reconciliation, education, and medication administration.
Care Coordination & Treatment Plan Management
Community Health Promotion
Professionalism
Knowledge, Skills, Abilities, and Other Characteristics :
Minimum Qualifications
Education : A Bachelor's Degree in Nursing from an accredited college or university and certification as a Registered Nurse (RN) required.
Licensure : Current license to practice as a Registered Nurse in the State of Arizona required.
Experience : Three (3) years professional Nursing experience required.
Equivalency : Any equivalent combination of education and experience that will allow the applicant to satisfactorily perform the duties of the job may be considered when filling the position except for completion of a BSN program, three (3) years professional nursing experience, and the possession of a license to be a Registered Nurse in Arizona.
Underfill Eligibility : An enrolled Community Member whom closely qualifies for the minimum qualifications for a position may be considered for employment under SRPMIC Policy 2-19, Underfill
Special Requirements
Prior to hire as an employee, applicants will be subject to drug and alcohol testing. Will be required to pass a pre-employment background / fingerprint check.
