Why M Physicians?
The Primary Care Clinic is centrally located within the Clinics and Surgery Center building allowing patients to see their primary care provider in the same building they may see their specialty care providers. Our clinic sees a large volume of patients providing care to all- from the routine physicals to the medically complex acute care visits. Our clinic is closely tied with the University allowing us to have a blend of dedicated faculty physicians working alongside internal medicine residents. This creates a collaborative environment for staff growth and daily learning.
What you will do as a Community Health Worker :
- Provide Direct Services such as wellness, health information, social services, disease prevention, chronic illness medication and social support. adherence to patients and providers
- May organize and / or facilitate support groups
- Refer and link to preventive services through health screenings and healthcare information
- Bridge the gap between communities and the health and social service systems by enhancing care quality by aiding communication between health system(s) and patient to clarify cultural practices and health information.
- Make referrals and coordinate services and enroll patients into programs such as health insurance and public assistance
- Partners with patient / caregiver / management in care / decision making.

Hours : Full Time, 40 hours a week, weekday day time hours

Compensation :
21.20 - 30.74 USD Hourly
