Right at Home Cedar Rapids is in need of an experienced and enthusiastic Home Health Nurse Manager to ensure our clients receive the highest quality home health care!  Our Home Health Nurse Manager  is a member of the Company’s management team who participates in planning, policy formulation and administrative decision making with a particular reference to the role, functions, and operations of the Company’s services.

ESSENTIAL FUNCTIONS OF OUR HOME HEALTH NURSE MANAGER :

• Maintains compliance to all Company clinical policies, procedures, and regulatory requirements.
• Evaluate and supervise all Company clinical staff for conformity with professional Joint Commission standards of home health care, and in accordance with state health statues.
• Ensures the quality of patient care and service by using specialized knowledge and judgment to assure patient care standards are in compliance with applicable Iowa State Nurse Practice acts, Company policies, and CMS requirements.
• Performs or supervises case management activities for individual patients, including but not limited to assessments, teaching and development and updating of the care plan.
• Audits charts on a regular basis to assure proper maintenance of clinical records to include OASIS documentation.
• Manages and enforces completion of annual compliance training.
• Coordinates arrangements with other healthcare and equipment providers to assure continuity of care and adherence to the patient plan of care.
• Provides for orientation and in-service education for clinical staff.
• Communicates with physician's offices, discharge planners, hospitals, skilled nursing facilities and families regarding the client’s care and status.
• Actively engages in training and new hire orientation to teach delegated nursing tasks.
• Reviews client / caregiver notes to ensure compliance and quality in documentation and care.
• Promotes a positive image of the Company within the office and in the community.
• Promotes Company to the community, participating in community programs with other professionals when necessary and forming alliances that are most beneficial to the Company.
• Documentation of care / issues / events in WellSky, Matrix Care, written notes in client / caregiver files as directed by state licensure guidelines, CMS regulations, and Company policies.
• Completes performance reviews of all clinical staff members as per office policy.
• Follows up with all Company complaints by both staff and client.
• May be required to travel on company business, particularly to Waterloo / Cedar Falls area.
• Be ready to serve as the Administrator in her absence.

KNOWLEDGE / SKILLS / ABILITIES / AVAILABILITY for a Home Health Nurse Manager

• Must be, at a minimum, a registered nurse (RN) with supervisory experience licensed in good standing in the State of Iowa.
• Home health care experience
• Read, write speak and understand English as needed for completion of job.
• Current CPR certification.
• Have a valid driver’s license and use of an insured automobile or access to adequate transportation.
• Serve in on-call rotation.
• Works in office setting and performs assessments in clients' homes.
• Computer experience with Microsoft Office Suite and EMR systems.
