Responsibilities :

• Conduct telehealth nutrition counseling sessions using an evidence-based, patient-centric approach to care, leveraging our technology platform.
• Demonstrate strong clinical and behavior change coaching skills, leading with empathy and use of motivational interviewing.
• Support a diverse patient population with a wide range of clinical conditions.
• Complete thorough and timely documentation for each session, including clinical and patient notes, in accordance with established standards of practice.
• Collaborate with the clinical team to provide patient-centered care, participate in monthly case reviews, and contribute to overall team success.
• Manage a caseload of 250+ members monthly through telehealth services, via phone calls and secure messaging, with a focus on building rapport to drive engagement.
• Ability to work independently and make decisions based on standard of care.
• Own the member experience and seek out support from team members
• Hold active licenses in 15+ states and credential with health plans as needed to meet client and organizational requirements.
• Work 40 hours per week, including at least one evening shift.
