HOME HEALTH NURSE LPN

Full Time

Job Description :

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Follows the plan of care appropriate to the patients diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates homecare services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.
• Accepts and transcribes physicians orders as directed.
• Forwards all clinical, progress, and summary notes to the Registered Nurse for coordination.
• Attends and participates in patient care conferences.
