Description :

Altra Homecare Group is actively searching for a RN - Home Healthcare Nurse. The RN will provide direct care for a team of clients in accordance with the state Nurse Practice Act and Agency policy.

QUALIFICATIONS :

• Graduate of an accredited school of professional nursing.
• Current license as a Registered Nurse in the state(s) of practice. Baccalaureate degree preferred.
• Minimum of one (1) year of nursing experience. Home care / Oasis experience preferred.
• Demonstrated strong and effective written and verbal communication skills and good interpersonal skills.
• Ability to deal tactfully with clients and the community.
• Demonstrated organizational and time management skills.
• Knowledge of home health regulatory and reimbursement requirements.
• Licensed driver with automobile insured in accordance with state and / or Agency requirements and in good repair.

ESSENTIAL FUNCTIONS / AREAS OF ACCOUNTABILITY

Directs, plans, and initiates appropriate action independently and responsively in home care situations.

Maintain accurate and timely documentation of patient care activities and observations.

Monitor patients’ conditions and progress, notify supervisor and physician of changes in conditions.

Administer medications, treatments, and other prescribed interventions.

Educate patients and their families on disease management, home care techniques, and lifestyle adjustments.

Coordinate with other healthcare professionals and agencies to ensure comprehensive patient care.

Promotes personal safety and a safe environment for clients and coworkers.

Performs job duties in accordance with Agency policies, procedures, and professional and community standards.

Respond to emergency situations in a prompt and professional manner.

Performs other job duties as assigned.
