Position Overview

Healthy Living at Home, LLC (San Jose) is searching for a professional and caring Registered Nurse Case Manager to provide case management, assessment, evaluation, direction, and care to our Home Health patients in the Greater San Joaquin County, Stockton, Lodi, and Galt, CA metropolitan area.

Essential Job Functions

• Complete an initial assessment of patient and family to determine home care needs.
• Provide a complete physical assessment and history of current and previous illnesses.
• Regularly re?evaluate patient nursing needs.
• Initiate the plan of care and make necessary revisions as patient status and needs change.
• Use health assessment data to determine nursing diagnosis.
• Develop a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
• Include the patient and the family in the planning process.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by the physician.
• Counsel the patient and family in meeting nursing and related needs.
• Provide health?care instructions to the patient as appropriate per assessment and plan of care.
• Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient.
• Act as Case Manager when assigned by the Clinical Supervisor / Nursing Supervisor and coordinate patient care for the assigned caseload.

Physical Requirements

Typical functions of this role include standing, sitting, and walking a majority of the time. Occasional kneeling, stooping, bending, or reaching may be necessary. The employee must be able to lift and / or move up to 10 lbs regularly and up to 40 lbs on occasion. Specific vision abilities required include close, distance, color, peripheral, depth perception, and ability to adjust focus.
