We offer in-house subacute rehabilitation services designed to support recovery after illness, injury, or surgery.
Position Summary
We are seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our interdisciplinary rehabilitation team. The SLP will assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders for residents receiving subacute rehabilitation services. This role involves direct patient care, care planning, and collaboration with therapy and nursing teams to optimize resident outcomes.
Responsibilities
Evaluate residents for speech, language, voice, cognitive-communication, and swallowing disorders
Develop and implement individualized treatment plans based on patient needs and goals
Provide therapy to improve functional communication and safe swallowing
Participate in interdisciplinary team meetings and contribute to care planning
Educate residents, families, and staff on communication and swallowing strategies
Maintain accurate and timely clinical documentation in accordance with state, federal, and facility guidelines
Monitor and report progress toward therapy goals
Ensure compliance with all applicable regulations and standards
Qualifications
Master’s degree in Speech-Language Pathology from an accredited program
Current Florida state licensure as a Speech-Language Pathologist (or eligible)
Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred; CFY supervision may be available
CPR certification
Previous experience in a subacute rehab or skilled nursing facility preferred
Strong interpersonal and communication skills
Ability to work effectively as part of an interdisciplinary team
