Registered Nurse - Inpatient Clinical Care Manager

Description :

At St. Joseph's Health the Registered Nurse Inpatient Clinical Care Manager assumes the responsibility and accountability for collaboratively managing the progression of patient's care with the physician and health care team utilizing evidence based best practice guidelines, a patient advocate who ensures optimal communication, appropriate use of clinical resources and management of patient flow.

What you will do :

• Attend SNAP huddles if needed and works collaboratively with unit staff and support team (RN, Physician / Clinical Affiliate, DPA, MSW, CDI, Rehab Services, etc.) to progress each patient's care towards discharge.
• Provides patient and family education regarding care plans, discharge instructions, and available resources.
• Documents case management activities accurately and thoroughly in the electronic medical record (EMR).
• Participates in quality improvement initiatives aimed at reducing length of stay, preventing avoidable readmissions, and improving patient outcomes.

Pay Range : $35.80 - $45.35

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
