job_description.job_card.job_descriptionProvide compassionate, evidence-based care for older adults with acute and chronic conditions, using sound medical judgment and consulting specialists as needed. Ensure compliance with all clinical, regulatory, and ethical standards, including HIPAA, CLIA, and accrediting bodies. Engage in all aspects of patient care : assessment, diagnosis, treatment, education, and care coordination, emphasizing prevention and outcomes. Maintain accurate, timely EMR documentation within 48 hours of each visit. Collaborate with hospitalists during inpatient stays and remain involved in patient care. Demonstrate professionalism, quality, and a commitment aligned with the Group s mission.

100 % Outpatient
- Monday-Friday schedule
- Geriatric patient load
- 12-15 patients per day
- Established patient base
- Full support Staff

BLS
2-3 references
DEA
BC or BE in Internal or Family Medicine
Experience with Medicare Advantage patients
