The client is an innovative US-based physicians network that has been delivering high-quality healthcare to elderly patients in assisted and independent living communities, group homes, and private homes for more than five years.

Our client has 50+ full-time healthcare clinicians (board-certified physicians, nurse practitioners, physician assistants, podiatrists, psychiatrists, psychologists, and wound-care specialists) who visit patients in the comfort of their own homes. They are also rapidly expanding the house-call network model with the patent-pending telemedicine platform via 4G / 5G smartphones and tablets.

Partner Nurse Practitioner / Physician Assistant

Essential Functions

• Diagnose and treat patients in assisted living / group homes and skilled nursing facilities, including House Call services for common chronic diseases via "in-person" House Call and / or telemedicine visits.
• Prescribe all necessary medications and treatments
• Perform comprehensive physical examinations of patients
• Develop and implement patient management policies and procedures
• Facilitate referrals to other healthcare professionals and medical facilities
• Communicate with collaborating physicians or specialists regarding patient care
