We are looking for a Case (Family) Manager to enhance the quality of client management, maximize satisfaction, and promote cost-effectiveness. The successful candidate will be accountable for the full client care cycle. Case (Family) Manager duties include assessing, planning, implementing, monitoring, and evaluating actions required to meet the child's health and human services needed. A certified Case Manager is a plus.

Take Note : This position will require you to obtain a full DFS County background and Reference check, Nevada Residence fee is $45, and out-of-state residents relocating to Nevada will have additional fees, TB, CPR, and internal testing before beginning your contract with the agency. We do not cover the costs for any of the aforementioned testing or clearances so some up-front costs exist.
Case (Family) Manager Responsibilities Include, but are not limited to :
Coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centeredHandling case assignments, drafting service plans, reviewing case progress, and determining case Plan of Care (POC)Helping clients achieve wellness and autonomyWe are looking for :
Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centeredHandle case assignments, draft service plans, review case progress, and determine case closureHelp clients achieve wellness and autonomyFacilitate multiple care aspects (case coordination, information sharing, etc.)Help clients make informed decisions by acting as their advocate regarding their clinical status and treatment optionsDevelop effective working relations and cooperate with their therapeutic team throughout the entire case management processArrange abeyances with social services, health, and governmental agenciesTake the extra mile and interact with clients and governmental agencies to keep track of their progress and to ensure satisfactionRecord case information, complete accurately all necessary forms, and produce statistical reportsPromote quality and cost-effective interventions and outcomesAssess and address motivational, physical, educational, and psychosocial issuesAdhere to professional standards as outlined by protocols, rules, and regulationsQualifications :
Proven working experience in case management, including as a Mental Health Care Manager or a related jobExcellent knowledge of case management principlesPrevious experience with psychological aspects of careEffective communication skills including effective use of Microsoft Office ProductsExcellent organizational and time management skillsFamiliarity with professional and technical emerging knowledgeProblem-solving skills and ability to multi-taskCompassionate with teamwork skillsOver the age of 21 and have exhibited work with families and children in excess of 5 yearsMust have a reliable vehicle - Auto Insurance must be in your name or as a designated driver to the policy.Valid Nevada Driver's LicenseAble to pass a background check and drug screening including MarijuanaAll training requirements for this position are held at our office located at 98. E Lake Mead Pkwy, Henderson, NV 89015Fingerprinting for Background check costs begin at $45 for Nevada residents. Out-of-state residents will have additional costs depending on the state.Education and Professional Work Experience Requirements :
MUST HAVE at minimum a Bachelor's degree in one of the following fields of study :
