About the Role
We're seeking an experienced Registered Nurse to join the OB Care Management team , supporting high-risk pregnant Medicaid members across Indiana. This role focuses on person-centered care planning, health education, and coordinating medical and social services to improve maternal health outcomes.
Key Responsibilities
- Assess member needs across physical, behavioral, and social health domains
- Develop, implement, and monitor individualized care / service plans
- Conduct 15–30 telephonic outreach calls daily
- Identify barriers and connect members to community resources (housing, transportation, baby supplies, etc.)
- Educate members on pregnancy, chronic conditions, and plan benefits
- Maintain compliant and timely documentation ( monthly audits must reach =94%
- Participate in daily huddles, team meetings, and OB care rounds
- Complete a structured 3-week onboarding and precepting program
Required Qualifications
Education :
Graduate of an accredited School of Nursing OR Bachelor's in NursingExperience :
2–4 years of clinical or care management experienceRequired : OB experience (Prenatal, Postpartum, L&D, Mother-Baby, or NICU)Licensure :
Active Indiana RN License or Compact RN LicenseSkills :
