ASHLIN Management Group is seeking a Case Manager to join our team.

Education and / or Experience
Bachelor’s degree in social work, psychology, sociology, counseling, or related service / science or health care related disciplines and one year of experience providing case management services (internship experience applicable). At least two years of work experience in a social service and / or human service field will be considered in lieu of a degree. Certification and / or licensure in relevant discipline (for example, certified addictions counselor) may also substitute for educational requirements.
Bilingual applicants will be recruited to ensure that culturally competent and linguistically appropriate services are provided to individuals and families.
Ability to work with clients in a caring and respectful manner and with understanding of and consideration for their unique circumstances
Ability to develop, evaluate, and implement a case management plan meeting all milestones. Ability to identify resources and services and link clients as necessary
Ability to complete written forms and reports accurately and in a timely manner
Effective at building rapport and trusting relationships
Ability to serve as a role model to clients
Effective communicator; good listening and coaching skills
Ability to perform comfortably in a fast-paced, deadline-oriented work environment
Ability to successfully execute many complex tasks simultaneously
Ability to work as team member, as well as independently
Computer literate and proficient at using Microsoft Office

Job Responsibilities
Coordinate all services and help ensure that the client is making meaningful progress toward increasing earnings and securing stable housing
Introduce clients to the process, review the - assessments, develop and amend Housing Stabilization Plans and DIRPs, help clients overcome barriers to housing stability and self- sufficiency.
Develop trusting relationships with clients assigned to them and monitor clients' progress on housing stability journey
Responsible for conducting outreach to referred clients, engage clients in 2Gen activities, re-engaging those clients who started to participate and subsequently stopped
Responsible for the engagement of clients and should identify goals and activities appropriate for housing stability, barrier removal and career-pathways.
Responsible for coordinating with TEP providers to identify appropriate education and occupational training programs in addition to employment opportunities for families
Meet with assigned clients as well as maintain case notes on all client contacts
