Overview

We are seeking a compassionate and motivated Registered Nurse (Home Health) to join our clients dedicated care team in Greeley Pennsylvania. This position focuses on delivering high-quality patient-centered care directly in the comfort of patients homes. The ideal candidate will bring clinical skill empathy and a strong commitment to improving patients independence and overall quality of life.

Key Responsibilities

Provide skilled nursing care and comprehensive patient assessments in the home setting

Develop and implement individualized care plans in collaboration with physicians and the healthcare team

Educate patients and families on health management medications and post-treatment care

Monitor patient conditions and adjust care plans to support positive outcomes

Maintain accurate timely documentation consistent with clinical and regulatory standards

Communicate effectively with the care team to ensure coordinated holistic patient care

Schedule and Shift Details

Monday through Friday 8 : 00am 5 : 00pm

On-call weekend rotation every 4 8 weeks (Saturday & Sunday 8 : 00am 5 : 00pm)

Flexible scheduling and minimal weekend work

This is a Pay Per Visit position

Travel

Local travel required within the Greeley area (travel reimbursement provided)

Qualifications

Required

Associate degree or higher in Nursing

Active Registered Nurse (RN) license in Pennsylvania

Minimum of 1 year of professional RN experience (home health preferred)

Current BLS or ACLS certification through the American Heart Association (training support available if needed)

Valid drivers license reliable vehicle and current auto insurance

Preferred

Prior experience in home health or community-based nursing

Strong clinical assessment communication and organizational skills
