Social Worker BH Admissions

South Bend-IN-46601-United States

Reports to the Admissions Manager. Utilizes clinical expertise to assess patients who may require mental health services and coordinates care across the continuum of treatment.

Key Responsibilities

Patient Assessment & Care Coordination

Perform initial psychiatric assessments.

Obtain initial insurance authorization when applicable.

Consult with physicians and healthcare providers to ensure effective implementation of care plans.

Communicate treatment recommendations from physicians, including community resources, follow-up care, and patient / family education.

Ensure completeness and accuracy of medical records.

Report patient outcomes to the Admissions Manager.

Assist with Social Work services on inpatient units as needed.

Quality Improvement

Compile and analyze utilization data to identify trends and support continuous improvement initiatives.

Patient Registration & Financial Coordination

Interview patients and / or collateral sources to collect demographic, physician, and insurance information.

Accurately register patient information in hospital systems and update financial records as needed.

Verify insurance coverage, obtain pre-certifications, and ensure documentation is complete.

Provide required Medicare documentation and complete MSP / ABN forms.

Assist the Patient Account Liaison (PAL) in securing payment arrangements and obtaining additional documentation.

Obtain all required patient signatures, including consent to treat and assignment of benefits.

Patient Admission Support

Notify assigned units of patient arrivals.

Prepare charts, ID bands, and medical record labels.

Arrange escorts to safely guide patients to their assigned unit.

Department & Organizational Responsibilities

Attend department meetings and remain accountable for shared information.

Complete mandatory education, annual competencies, and health requirements within established timeframes.

Maintain active licensure / certification in good standing.

Maintain current BCLS (CPR) certification and any department-specific certifications.

Adhere to universal precautions, protective equipment protocols, and ergonomic safety practices.

Comply with regulatory standards and departmental policies.

Work overtime or additional shifts as needed to support departmental operations.

Education & Experience

Master’s degree in Social Work or Counseling required.

Minimum of two years’ experience conducting psychiatric assessments preferred.

Knowledge & Skills

Strong clinical knowledge of procedures and interventions for targeted patient populations.

Effective communication and interpersonal skills (verbal, written, and listening).

Ability to prioritize and manage multiple tasks independently under pressure.

Resourceful in identifying and utilizing support services.

Proficient in computer systems and electronic medical record documentation.

Working Conditions

Functions in a variety of environments with frequent changes in patient needs and departmental demands.

Physical Demands

Requires physical ability and stamina necessary to perform the essential functions of the position.
