The Opportunity : THIS WORK MATTERS!

At Multnomah County Public Health we provide needed health care services to adults at risk of infections and other public health priorities. We provide services aimed at improving the quality of life for people who use drugs are at risk of sexually transmitted infections vaccine-preventable illnesses and more.

The Advanced Practice Clinician (APC) will work primarily at the Harm Reduction Clinic (HRC) to provide effective and efficient evaluations diagnosis treatment plans and prevention services. Direct clinical work will be focused on evaluation and treatment of wounds and abscesses associated with substance use as well as HIV / STD testing and diagnosis and STD treatment. This role will work in cooperation with the Multnomah County STD Clinic Primary Care (Integrated Clinical Services) Pharmacy and Corrections Health. This role will also enhance and / or create referral mechanisms for patients seeking medical services beyond the scope of the HRC.

The HRC is open two days per week (Mondays and Thursdays). The front of the space is centered around a Syringe Access Program (aka Needle Exchange). Non-clinical staff are focused on creating a welcoming environment for participants by providing some basic nutrition referral services naloxone fentanyl test strips and safer injection supplies.

Required training will be across the HRC and the Multnomah County STD Clinic located at the Gladys McCoy Building in Portland. Priority clinical services at HRC are wound / abscess care STD diagnosis and treatment and MSR at the HRC and STD diagnosis and treatment at the STD Clinic as needed and if schedule allows. Secondary responsibilities include collaborating with our medical director on clinical policies and workflows. We aim to have the clinic open for direct services for a third day each week once our capacity improves.

The ideal candidate for this role will be able to work independently in the assessment and care of HRC clients with or without clinical support from a medical assistant or nurse. They will perform phlebotomy collect specimens perform basic microscopy administer injections perform abscess incision and drainage and other wound care and complete their own documentation in the electronic health record (Epic). Training will be provided as needed for all of the above expectations. They are adaptable and able to perform in a clinical environment that varies from day to day in both the volume and health status of clients. They will be open to providing or referring for Medication Supported Recovery (MSR) as well as HCV diagnosis and treatment.

The APC must consistently demonstrate effective positive and respectful communication with patients and staff including : active listening empathy courtesy cultural sensitivity and the ability to promote modification of health behaviors with clients.

As the Advanced Practice Clinician other duties will include but are not limited too :

Clinical Care

The Advanced Practice Clinician formulates appropriate diagnoses and implements treatment plans based on Harm Reduction Clinic (HRC) practice guidelines Health Department policies and community standards of practice as well as State and MCHD stage-based behavioral counseling guidelines and training.

Perform sexual health history physical examination and specimen collection as outlined in HRC Program Multnomah County Public Health and state clinical guidelines as appropriate.

Orders and interprets laboratory studies including on-site provider performed microscopy for trichomonas yeast infections bacterial vaginosis.

Performs venipuncture as needed. Phlebotomy training is available as needed.

Performs diagnosis and treatment of genital and sexually transmitted infections (STI).

Performs wound care including care of skin and soft tissue infections as well as chronic conditions such as venous stasis ulcers.

Prescribes medications such as HIV Pre-Exposure Prophylaxis (PrEP) doxy-PEP contraceptives (including emergency contraceptives) and antibiotics per Program clinical guidelines.

Provide injection medications as outlined in HRC clinical practice guidelines.

Provide or facilitate ultra low barrier medication access or referrals to support recovery and reduce risk of overdose.

Provide health education to patients.

Provide follow-up patient consultations and telehealth visits.

Develop and direct patient-centered care plans with the assistance of other disciplines within the care team.

Maintains exam room cleanliness and supply in coordination with HRC staff.

Refer patients to other clinics when beyond scope of practice.

Consultation and Care Coordination

This position provides consultation to nursing staff Harm Reduction Clinic (HRC) program staff Health Department Clinics and community providers as indicated for continuity of client care. The Advanced Practice Clinician assesses and counsels patients regarding additional care needs including preventative healthcare specialty or rehabilitative services or high risk / acute care. Provides appropriate advice on the management of STD to county or community health care providers following HRC and STI Program clinical guidelines and in accordance with Centers for Disease Control and Prevention (CDC) treatment guidelines.

Make appropriate referrals to care including specialty and rehabilitative services in coordination with the patient and care team.

Provide direction to Community Health Specialists Community Health Nurse and other staff to implement aspects of care plans within their scope and training

Partner with the Health Officer / Medical Director and Harm Reduction Clinic Coordinator to coordinate STI-related care between the two public health clinics.

Assist in developing and implementing protocols procedures and other teaching tools for employees.

Participate in precepting students volunteers and new staff when appropriate.

Partner with the Senior Harm Reduction Specialist and Corrections Health to enhance and improve MSR access.

Provide in-service training to staff as a group or individually as requested.

Documentation and Reporting

The Advanced Practice Clinician is responsible for the timely and accurate documentation of client care through the use of electronic health record charting.

Review client records for needs for medications treatments education or referrals related to HIV STIs skin infections substance use disorder or Hepatitis C.

Document appropriately and clearly in the electronic health record (EHR) system.

Utilize panel based information technology tools to prioritize and direct proactive care for HRC clients.

Apply division department and general healthcare requirements for patient privacy (such as HIPAA) and other compliance standards.

Salary / Pay : Please be advised that the pay range listed for this position is intended to provide general guidance on the earning potential for the role. However actual compensation will be determined in accordance with the Oregon Equal Pay Law and will take into account factors such as the candidates relevant experience and education. Candidates should expect that initial offers will be made within the listed pay range and may not be at or near the top of the range.

Minimum Qualifications / Transferable Skills

Education / Training :

Masters Degree in Nursing AND one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position OR

Masters Degree in Physician Assistant Studies AND one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position

Certification Requirements :

License required to practice medicine as a Nurse Practitioner as outlined by Oregon Revised Statutes (ORS) 678.375 OR

License required to practice medicine as a Physician Assistant as outlined by Oregon Revised Statutes (ORS) 677.515

BLS certification required

DEA License
