PA Licensed Behavioral Health Case Manager

A company is looking for a Case Manager in Behavioral Health (Eating Disorder) - remote.

Key Responsibilities :

Telephonically engages members to identify barriers to care related to Social Determinants of Health

Facilitates the development of care plans by identifying member strengths, needs, and goals

Serves as a liaison within the member's health care team, especially in complex behavioral health situations

Qualifications :

Active PA Registered Nurse (BCBA preferred), LCSW, LSW, or LPC required

3-5 years post licensure experience in an integrated care or health care setting

Strong understanding of Social Determinants of Health and Motivational Interviewing techniques

Ability to work independently and resolve complex behavioral, medical, and psychosocial issues

Exceptional organizational skills and ability to meet individual productivity goals
