Scheduler
Provides scheduling and supervision of Personal Care Services for private pay clients. Schedules home health aides for skilled services clients. Works under the supervision of the Manager or designee. Supervises personal care providers and homemakers.
Examples of duties and responsibilities include :
- Orienting personal care and homemaker staff to agency policies and procedures.
- Arranging and documenting training.
- Informing staff of policies concerning advance directives and emergency procedures.
- Scheduling staff and notifying clients of changes.
- Performing written assignments of duties on a client-specific basis.
- Meeting and conferring with staff as necessary.
- Receiving and reviewing referrals of clients.
- Performing initial assessment visits to client homes, assigning caregivers and completing assessment and plan of care forms.
- Performing supervisory visits to clients' homes at least every three months or more frequently as needed for problem resolution; validating skills of staff and client-specific or procedure-specific training of staff; observing condition and care of client and assessing clients' satisfaction with services.
- At least one of the assigned personal care staff must be present at supervisory visits at least once every three months.
- Providing monthly supervision of client care via telephone or other appropriate contacts with client and / or family / care givers.
- Providing first level investigation of complaints and critical incidents.
- Counseling staff on difficult cases and potentially dangerous situations.
- Communicating with the case managers, the physician and other providers on the care plan as necessary to assure appropriate and effective care.
- Overseeing staff record keeping.
- Performing other related functions as required.
- Maintaining confidentiality in all aspects of job performance.
- Performing all job functions in compliance with all appropriate local, state and federal laws, regulations and professional standards.
- Attending and participating in staff, education, and agency committee meetings as appropriate.
Minimum qualifications :
Education : High School diploma or equivalent.Experience : A minimum of one year of scheduling experience is preferred. Home care experience is preferred.Knowledge, skills, and abilities :
Ability to guide and assist staff members in fulfilling their responsibilities.Ability to communicate effectively both orally and in writing.Knowledge of principles and procedures of personal care and safety practices in the home care setting.Understanding of family interactions and aging process.Ability to communicate effectively, verbally and in writing.Ability to establish and maintain effective working relationships with clients and families, staff members, and the general public.Special requirements :
Valid, current state driver's license.Access to a reliable automobile.Exposure / risk category : OSHA. Tasks involve no occupational exposure to blood, body fluids or tissue.
