Licensed Social Work Navigator

A company is looking for a Navigator, Helpline. Key Responsibilities Provide individual assistance to patients, survivors, and families to address barriers Deliver services via telephone and chat, utilizing the CSC Proactive Navigation model and psychosocial assessments Assess callers in crisis and connect them to appropriate resources and care Required Qualifications, Training, and Education Master's degree in social work At least two years of experience in oncology patient navigation or social work Social Work license in good standing from the state of residence Oncology social work certification preferred but not required Navigation certification preferred but not required
