Job Description :

As a Registered Nurse (RN) Case Manager specializing in hospice at Lifted Hospice, you will play a pivotal role in providing patient-centered care with a strong emphasis on compassion and dignity. Your responsibilities will include administering skilled nursing care for clients of all ages in their place of residence, coordinating care with the interdisciplinary team, patient / family, and referring agency, and assuming responsibility for the coordination of care.

Key Responsibilities :

• Comprehensive Patient Assessment : Conduct thorough assessments of patients' physical and emotional needs, tailoring care plans to meet individual requirements.
• Expert Hospice Care : Provide expert hospice care and symptom management to patients, ensuring their comfort and enhancing their quality of life during this sensitive time.
• Collaborative Approach : Collaborate closely with the interdisciplinary healthcare team, patients, and families to ensure holistic and comprehensive care.
• Meticulous Documentation : Maintain accurate and comprehensive medical records, adhering to regulatory guidelines and standards.
