We are actively seeking a compassionate and skilled Intake Coordinator to join our team full-time. This role is essential to our mission providing therapy crisis support and service coordination for mental health clients in a variety of settings. As a Intake Coordinator with Carolina Outreach you will be managing and coordinating incoming referrals to ensure timely and efficient access to services.

Compensation : $20.57 - $23.35

Position Description :

• Receive review and process incoming referrals from various sources ensuring all required information is complete and accurate.
• Communicate promptly with referral sources clients and families to gather intake information and explain the admission process.
• Coordinate and schedule intake appointments assessments and evaluations with appropriate clinicians and programs.
• Assign clients to appropriate providers or programs based on clinical needs location and service availability.
• Maintain accurate and timely documentation of referral and intake activity in the electronic health record (EHR) and referral tracking systems.
• Collaborate with clinical and administrative teams to ensure efficient handoff and continuity of care from intake to service delivery.
• Track referral outcomes monitor pending cases and follow up to ensure timely resolution.
